 Launch your Adult Foster Care Program in Massachusetts with CARF Accreditation

If you’re planning to open an Adult Foster Care (AFC) or Group Adult Foster Care (GAFC) program in Massachusetts, you’ll need to meet MassHealth enrollment requirements and complete national accreditation. MassHealth requires AFC/GAFC provider agencies to be accredited by an accepted accrediting body (including CARF) as a condition of provider eligibility.

Massachusetts Requirement: Accreditation Is Mandatory for MassHealth AFC/GAFC Enrollment

MassHealth requires AFC and GAFC provider applicants to be fully accredited before becoming a provider, and accreditation must be maintained to remain eligible. (

MassHealth’s provider eligibility regulation requires the provider agency to be accredited by CARF, COA, NCQA, or another nationally recognized accrediting organization acceptable to MassHealth, and to provide evidence of accreditation in the form and format MassHealth requires.

New Massachusetts Update: “8 Members” at Accreditation/Re-accreditation

MassHealth adopted an emergency amendment (effective September 12, 2025) adding a provider eligibility requirement: at the time of accreditation or re-accreditation, the provider must be delivering services to at least eight (8) individuals.

What that means for a new provider agency:

  • Your policies and systems must be accreditation-ready and Massachusetts-ready

  • Your service delivery model must be operational and functioning (not just written on paper)

  • Your launch plan must account for enrollment readiness and capacity so you can meet the 8-member threshold at accreditation/re-accreditation 

Massachusetts AFC Compliance: What Your Program Must Be Built to Do

MassHealth’s AFC regulations require a provider agency to maintain compliant policies, allow periodic inspections, and operate in accordance with 130 CMR 408.000 and MassHealth administrative and billing rules. (

And operationally, AFC programs must be staffed and structured to meet Massachusetts requirements, including a Multidisciplinary Team (MDT) with defined roles and qualifications (such as a Program Director, RN/LPN, and Care Manager).
