Why Timing Matters
Submitting your CARF survey application at the right time ensures that your organization receives its accreditation decision without gaps or unnecessary delays. Since CARF uses a structured scheduling system, missing the deadline can push your survey date back significantly, affecting operations and compliance.

Note: CARF does not issue July expirations since its standards manuals take effect on July 1 each year. Organizations with large surveys may need to submit applications earlier.
We recommend submitting your application at least 10 business days before the listed deadline to prevent processing delays. If any details change after submission, CARF must be notified immediately.
Step-by-Step Application Process
Step 1: Submit Your Survey Application
Start by completing your CARF survey application via Customer Connect, CARF’s online system. Be prepared to provide:
Leadership and program information
Details on services seeking accreditation
Service delivery location(s)
Supporting documentation
Step 2: Invoice and Scheduling
Once the application is reviewed, CARF will issue an invoice for the survey fee. After payment, the survey will be scheduled based on availability.
Step 3: Survey Team Assignment
CARF assigns experienced surveyors based on your program type. You’ll receive confirmation of your assigned team at least 30 days before your scheduled survey.
Step 4: The On-Site Survey
Surveyors visit your organization to:
Review policies and procedures
Interview staff and persons served
Assess service delivery and compliance with CARF standards
Surveys typically last 2-4 days, depending on your organization’s size and complexity.
Step 5: Accreditation Decision & Report
Approximately 6-8 weeks post-survey, CARF will issue an accreditation decision along with a written report outlining strengths and recommendations.
Step 6: Quality Improvement Plan (QIP)
Organizations receiving recommendations must submit a QIP within 90 days of their accreditation notification, detailing how they will address the suggested improvements.
Step 7: Annual Reporting for Three-Year Accreditation
Organizations awarded Three-Year Accreditation must submit an Annual Conformance to Quality Report (ACQR) each year to maintain compliance.
