A Walk Through Time and Challenge in Portsmouth, Ohio
During my recent trip to Portsmouth, Ohio, I conducted a CARF survey with a local organization striving for accreditation and a higher standard of care. What I didn’t expect was how much I would learn about the city itself—its rich history, its struggles, and its resilience.
Portsmouth, platted in 1803, flourished as a river town and industrial hub. It later gained a place in history as a key stop on the Underground Railroad. Its floodwall murals vividly portray these chapters, showcasing everything from industry to civil rights to community pride.
However, in recent decades, Portsmouth faced another defining moment: the devastating opioid epidemic.
The Opioid Crisis and Community Resilience
During my time conducting the CARF survey, conversations with staff and community members inevitably turned to the crisis that put Portsmouth in national headlines. Known as the “pill mill capital” in the early 2000s, Scioto County saw alarming numbers—at one point, an average of 123 pain pills per person were distributed annually.
By 2010, the county declared a public health emergency. Local leaders formed coalitions, shut down illegal operations, and began rebuilding trust in healthcare and human service systems. Today, Portsmouth represents not just hardship but transformation, with recovery programs and accredited organizations leading the way.
